- Two (2) dental cleanings (adult/child prophylaxis/periodontal)
- Two (2) Doctors Examinations
- One (1) Emergency exam with X-rays
- 1 set of required X-rays including: Bitewing (4 films)
- Oral hygiene instructions
- Fluoride 2 applications per year (no age restrictions)
Total Value up to $650.00
All other dental procedures at a 15% savings off standard fees (written financial arrangements are required before treatment is scheduled).

LIMITATIONS & EXCLUSIONS
- This program is designed to aid our patients in affording dental care. It is not a Dental Insurance Plan.
- This plan is for established patients of Doug Worful DMD PC only.
- Alpharetta Smile Care is not a licensed insurer, health maintenance organization or underwriter of health services.
- This plan cannot be used in conjunction with any other coupons, offers, dental insurance, discounts, or dental plans.
- This plan does not apply anywhere else other than Alpharetta Smile Care.
- This plan cannot be used for services or injuries covered under workman’s compensation or for dental care that is covered under automobile medical insurance.
- Rates and benefits are subject to change annually.
- Payments are due at time of services.
- Botox and Fillers are not included in the 15% discount fee.
GUIDELINES
- Once purchased, there is a one (1) year term for your savings plan and shall expire after 365 days.
- Payment in full for the savings plan is necessary prior to using your savings plan.
- NONREFUNDABLE: No refunds will be issued once the plan has been utilized. (Your very first appointment).
- Your savings plan is non transferrable.
- The patient portion after the discount is due the day of service.
